The program includes random selection, identify the nearest collection sites or provide on-site collections (available in specific areas), MRO Services, SAMHSA certified laboratory and statistical reports (as required by D.O.T. Management Information System M.I.S.).

DAT Express Test will provide (for a fee) Supervisor Substance Abuse Awareness Training.

Additionally, a program manual is supplied to include the necessary forms and updated information for the successful implementation and management of your program.

Express Packs

Express Packs are a sealed collection kit that contains a split specimen collection kit, D.O.T. chain of custody form, a courier airbill and courier shipping bag. These packs are kept with the driver in the vehicle at all times. Therefore in the event of a random or post accident test on the road will eliminate the possibility of errors during the testing.

All charges can be charged to your credit card, or with approval be billed net 30 days.

• DOT 5 Panel Pre-employment Test $75.
• DOT 5 Panel Test $45.00 Clinical / $55.00 On-site testing in Metro Phoenix Area
• DOT 5 Panel Test $45.00 / Collection fees may apply outside of Metro Phoenix Area
  (DOT 5 Panel Pre-employment Drug Screen exempt if company can provide a current pre-employment drug screen)
• MRO (medical Review Officer) Included
• ALCOHOL TESTING Breath Alcohol (EBT) or Saliva Alcohol (STT) $35.00 per test
